For this assignment, you are responsible for finding and interviewing an individual who has been diagnosed with one of the chronic diseases or disabilities.

The purpose of this assignment is to help you gain some insight into what it is like for someone who has to deal with a chronic disease or disability

Your paper must be typed and double-spaced using a 12-point font.

Include the date, the name of the person being interviewed and the specific disease or disability s/he has.

You must give a list of your interview questions. If desired, you may list the question, followed by the individual’s response but there must also be a portion of the paper where you discuss and reflect on what you learned from this experience.

There should be evidence, as the paper is read, that you have completed the textbook readings about that particular disease state in order to gain full credit for this assignment.

It is important to find out if the person you are interviewing exercises. Has s/he ever gone through exercise testing or received an exercise prescription? If the person does not exercise, explore this area. Does the doctor recommend exercise or is it contraindicated at this time? What barriers might be keeping the person from exercising if it has been recommended?

Your paper should be grammatically correct in every way with no spelling errors.
