Health promotion incorporates the individual community society and the global population. Nurses have the privilege of impacting health at each of these stages. The goal of the program is to assist individuals in developing and/or improving existing life skills through an array of services that maximizes independence enhances quality of life and encourages community integration. Each person enrolled within the program is provided with an individualized plan that addresses his or her dreams interests and needs with an emphasis on three components: living skills personal development and therapeutic intervention
6. Population served.
i. The agency serves both male and female 18 years and older.
ii. Individual that has a documented developmental disability or mental illness or referred by the department of Rehabilitation Services.
iii. Individual who is able to self-transport or have transportation provided for Mental Health Day or Career Services.
iv. Individual that meets funding requirements of the Department of Human Services or be able to pay the cost of services (e.g. complete PAS screen for Developmental Development services apply for Medicaid receive Department of Rehabilitation Services funding etc.) or be able to cover the costs of services through personal means.
7. Cost of service to client.
The cost of services is based on standard rates from The Illinois Department of Human Services and covers all applicable program services and fees. The minimum standard cost per client is about $10/day depending on individual needs. For example some clients needs cannot be safely met with the level of staff support available in the program hence will require a personal assistant and an increase in the rate.
8. Types of personnel and their roles. (Is there a nurse employed?)
i. Agency Director (Qualified Intellectual Disabilities Professional QIDP): Responsible for the administrative influence to ensure that mental health services employ sufficient mental health professionals. The director is also the person in charge of maintaining records and making reports as needed. Normally much of this work will be completed by other staff members in a mental health service but the director retains final responsibility for making and keeping records and responding to requests for information from the department of health. In addition the director is able to influence operational and staffing decisions within the agency to operate effectively.
ii. Direct Support Professional (DSP): Assist clients in maintaining safe and healthy lifestyles and monitor for changes in their general health appearance behavior and patterns of activities. Also the DSP is responsible for monitoring and maintaining a clean safe and secure environment for clients at all times. In addition the DSP plan and support clients in daily activities which include but not limited to recreational activities learning activities physical fitness activities sensory activities and money management.
iii. Community Inclusion Specialist: Responsible for making the clients doctors appointment transport them to and from their various hospital appointment as needed.
iv. There is no nurse employed on site but there are nurses that visits clients in their homes as needed.
